Your Voice Matters. We Want Your Feedback!

The purpose of this assessment is to learn from you what is working and what is not working with your services, and to hear your concerns. We will focus on barriers to accessing care, understanding the needs of the community, and recommendations for improvement. 

This assessment is for families, caregivers, and people we serve.

Participants will receive a $35 Meijer gift card, but space is limited. First come, first served!
